Scientific Poster Session

Important Information as follows:
* Required Fields * All presenters must be registered for the GNYDM before completing this application. After you register you will receive a confirmation email which included your Registrant ID number which you will need to complete the application
* All clinicians must be onsite to present their poster during the entire duration of the session (Sunday, December 1, 2024 from 10am-2pm)
* An appropriate title for each demonstration must be included
* Each participant must complete their mailing address, telephone number and email on the application
* Please ensure that all names and poster titles are correct and that the forms are filled out in their entirety
* All applicants will need to submit a completed template in order for their Poster Board to be produced by our office and to participate in the Scientific Poster Sessions
* I agree to have my poster displayed online at the conclusion of the 2024 GNYDM.

Various sessions of the Greater New York Dental Meeting may be photographed or video recorded for marketing purposes. Your permission is required to record these sessions
* I authorize the Greater New York Dental Meeting and its designees to photograph or video record pieces of my presentation. I understand that this footage may be for promotional usage.
